Cancer Chinese Internal Medicine
Chinese medicine therapies in cancer treatment are very common in the East. Although it is usually classified as a form of complementary and alternative therapy in the West, Chinese medicine is an independent medical profession in Hong Kong and mainland China. It has a different perspective in understanding health and diseases compared with Western medicine. In oncology practice, whereas Western medicine focuses on direct tumour eradication by surgery, radiation therapy and systemic therapies, Chinese medicine focuses on restoring body balance and enhancing the body's defences (immunity), in addition to some cytotoxic herbal therapies. Most often patients, especially those in the East, receive both treatments. Chinese medicine is also commonly used to reduce side-effects from chemotherapy or radiation therapy, to aid recovery after an operation, to palliate symptoms and to address survivorship issues. However, this raises concerns of drug-herb interactions and toxicity in combination therapies. Commonly used Chinese medicine treatment modalities include acupuncture, moxibustion, diet therapy, prescribed Chinese medicine herbal decoction, single Chinese medicine herbs or supplements and tai chi. Although there is an increasing trend of Chinese medicine use in cancer patients in both the East and the West, the scientific evidence of safety and efficacy is often questioned by oncologists. This article reviews the current evidence in different Chinese medicine therapies in cancer management in both the East and the West.    Main syndromes and characteristics

Cancer is characterized by abnormal mass in the visceral tissues (leukemia is an abnormal proliferation of the number and quality of the white blood cell series in the bone marrow and other hematopoietic tissues). The occurrence of mass is mainly due to the intertwining of qi stagnation, phlegm coagulation, dampness stagnation, blood stasis, toxic accumulation, etc., which becomes a tangible mass after a long time. Cancer patients have more deficiency of the hormone body, and cancer lesions consume the body's Qi-blood body fluid, so the middle and late patients have more Qi-blood deficiency, Yin and Yang deficiency and other pathologic changes. The main syndromes are as follows:

Stagnation of vital energy

1. The main pulse disease of the viscera or corresponding parts of the fullness, pain, thin greasy fur, pulse string.

2. The main symptoms of Qi stagnation are distension and pain. The pain is distension rather than tingling. The parts of Qi stagnation can wander and stop from time to time.

cuspidor

1. Main pulse disease cough phlegm, dizziness, phlegm nucleus, limb joint pain, diseased viscera appear lumps, white fur, pulse slippery.

2. Syndrome characteristics Sputum turbidities and accumulated lung manifestations of cough sputum, phlegm Mengshen see delirium, phlegm stagnation muscles and bones for phlegm nucleus, phlegm blocked meridians see limb joint pain, spittoon congealing in the zang fu organs or with wet accumulation, blood stasis and other intertwined mass, mass texture is not hard, can be accompanied by pain.

Damp stagnation

1. Main pulse diseases cough phlegm, loss of appetite, lethargy, abdominal distension, diarrhea, unfavorable urination, white greasy or white slippery fur, pulse moistening.

2. Syndromic characteristics of dampness in upper jiao leads to cough and phlegm, in middle jiao leads to loss of appetite, lethargy, abdominal distension and diarrhea, and in lower jiao leads to unfavorable urination.

Blood stasis

1. Main pulse disease lesions pain, pain is fixed, or petechial lumps, or fever, dark complexion, skin nails, purple tongue, or ecchymosis, pulse astringent or string.

2. The most common symptom of blood stasis is pain, and its pain is in a certain place, mostly stinging pain, long-term pain does not heal, and repeated occurrence is characterized.

Toxic accumulation

1. Main pulse disease fever, bleeding, lesion site red, swelling, heat, pain, constipation, urine short red, tongue red fur yellow, pulse number.

2. Syndrome characteristics Poison is the extreme of fire, so it is characterized by the prominent signs of fire.

Deficiency of vital energy

1. Main pulse disease mental wilt, fatigue, shortness of breath, dizziness, spontaneous sweating, easy to catch a cold, white face, pale tongue, thin white fur, weak pulse.

2. The main syndromic features are a series of qi depletion and zang fu function decline. The focus of symptoms varies with the different viscera of the disease.

Deficiency of blood

1. The main pulse symptoms are dizziness, fatigue, insomnia, forgetfulness, palpitation, palpitation, palpitation, weak pulse, pale or yellow face, pale lip nails, pale tongue, white fur, weak pulse.

2. The syndrome is manifested as a series of symptoms of blood deficiency and loss of nutrition, zang-fu function decline. The main difference between Qi-deficiency and Qi-deficiency is that this syndrome has prominent manifestations of camp blood deficiency, such as the facial color is not bright, the lip nail is not honorable, and the cause of excessive blood loss often exists.

Deficiency of Yin

1. Main pulse symptoms dry mouth and dry lips, five upset heat, hot flushes and night sweats, upset insomnia, waist and knees sour and soft, dry skin, dry stool, red tongue less fluid, thin pulse.

2. Syndrome features This syndrome is manifested as a series of Yin fluid deficiency, loss of moisture-moistening symptoms. Yin deficiency often produces internal heat, and more accompanied by the appearance of deficiency heat.

Deficiency of Yang

1. The main pulse disease is lethargic sleepiness, little qi and lazy words, cold shape and cold limbs, palpitation from perspiration, poor appetite, limb edema, pale or yellow face, cold waist and knee pain, impotence and spermatosis, loose stool, clear and long urine, pale and fat tongue, teeth marks, white fur, and delayed pulse.

2. The syndrome is manifested as a series of symptoms of Yang qi deficiency and failure, loss of warm. Yang deficiency produces internal cold, and common deficiency cold signs.

A summary of the pathogenesis

1. Qi stagnation can be caused by many reasons, such as emotional discomfort, eating disorders, feeling external evil, phlegm turbidity, blood stasis and so on. Due to the block of qi and the obstruction of qi and blood movement, fullness and pain appear in the diseased viscera or parts.

2. Due to various reasons such as external or internal injury, the lung is lost to the diffusion of body fluid, the spleen is lost to the transport of body fluid, and the kidney is lost to the warm steaming water, which can lead to turbidness of phlegm. There are a variety of clinical manifestations depending on the location of the lesion, which are characterized by cough sputum, phlegm nucleus and mass in this chapter.

3. Due to external or internal injury and many other reasons, wet stagnation leads to lung, spleen, kidney dysfunction, water and liquid metabolism disorders, so that body fluid is stopped and water and wet disease. There are a variety of clinical manifestations depending on the location of the lesion. In this chapter, it is characterized by the changes of spleen and stomach caused by dampness, such as loss of appetite, lethargy, abdominal distension and diarrhea.

4. Blood stasis can be caused by a variety of reasons, such as uneasiness of blood stasis, eating disorders, feelings of external evil, injury of falling and falling, and positive deficiency after a long illness. Due to blood stasis, and even vein stasis, pain is not generally caused, and cause pain, or obstructed fever, long-term accumulation. In this chapter, the lumps are its outstanding characteristics.

5. Toxic accumulation is caused by external heat evil, or internal spittoon coagulation, dampness stagnation, blood stasis and other pathological products obstructing qi mechanism, stagnation and heat, or internal and external evil, making heat evil hyperactive and toxic. Heat evil is hyperactive, so fever, forcing blood to move and bleeding, phlegm, damp stagnation, blood stasis, it can form a lump.

6. Qi-deficiency is mainly due to eating disorders, water and grain are not full, and the source of qi is insufficient. Or due to serious illness, old age and excessive fatigue, so that the function of zang-fu weakened, insufficient qi metaplasia. Due to the lack of positive qi, it can not normally play the role of qi promotion, solid, warm, Wei and other functions, and the performance of fatigue and weakness, mental wilt, spontaneous sweating, easy to catch cold and other diseases.

Seven. Blood deficiency is often caused by excessive blood loss, spleen and stomach weakness, good nutrition, persistent illness, and blood metaplasia disorders. Due to the deficiency of Ying blood, the loss of zang fu meridians in mutual care, and the performance of dizziness, God fatigue, pale yellow, lip armor and other diseases.

8. Yin deficiency is caused by dryness and heat injury to Yin, or chronic illness injury to the yuan Yin of the kidney. Due to the lack of Yin essence, it is lost in the function of nourishing the viscera and meridians, so there are dry mouth, dry lips, dry skin and other diseases, Yin deficiency is Yang hyperactivity, so there are five upset heat, hot flushes and night sweats and other deficiency heat symptoms.

9. Yang deficiency is mainly caused by qi deficiency further development, qi loss and Yang. Due to Yang Qi deficiency and decline, lost in warm, and the performance of god tired lie, less qi lazy, cold limbs cold and other deficiency cold symptoms.

Treatment point

Cancer belongs to a kind of disease of positive deficiency and negative excess, so the basic principle of treatment is to fuzheng and dispel evil, attack and tonifying. It is necessary to combine clinical data such as medical history, course of disease, four diagnoses combined with laboratory examination, comprehensive analysis, syndrome differentiation and treatment, so as to "treat reality when looking at deficiency, supplement deficiency and don't forget reality". The method of Fuzheng is mainly based on the different emphasis of positive deficiency, and combined with the main lesions of the zang-fu organs, respectively adopt the treatment of supplementing qi, supplementing blood, supplementing Yin and supplementing Yang. Dispelling evil mainly adopts the methods of regulating qi and promoting qi, resolving phlegm and dispersing nodules, promoting blood circulation and removing stasis, clearing heat and detoxifying.
